Established in 1971, Gastroenterology Associates
of Southeastern Virginia, has spent many years providing outstanding
healthcare to patients all over the Tidewater area and northeastern
North Carolina. Through the years, we have built strong relationships
with many referring physicians and recommend that all patients should
have a primary care physician to oversee their healthcare. Patients,
however, are also welcome to schedule their own appointments.
To schedule an office appointment, simply

may request appointments online any time of day by visiting our
EMR online patient
portal.

What should you bring?
• Please bring the following to all appointments:
• Photo identification (driver’s license, military id
or state issued photo id card)
• Insurance card(s)
• Medications or current medication list
• Any referrals or authorization required by your insurance.
• Any applicable co-payment required by your insurance (most
major credit cards accepted)
If you are unable to keep any appointment,
office or procedure, we will be glad to reschedule your appointment,
however we request that you give us enough notice so we may provide
another patient the opportunity to be seen. For office appointments,
we request 24 hours notice. For procedure appointments, we request
72 hours notice. Failure to keep a scheduled office appointment
or give adequate notice may result in a financial penalty.

Open Access Colonoscopy:
An Open Access Colonoscopy is simply a method of scheduling a colonoscopy
without the requirement of being seen in the office first.
If you require a screening colonoscopy
due to age or a family history of colon cancer or colon polyps,
you may request an Open Access Colonoscopy.
Just download this form, Open
Access Colonoscopy request form , and give it to your primary
care provider who can fill it out and fax to us. We will then contact
you to set the appointment and send you preparation instructions.
